The aging face presents a therapeutic challenge that requires understanding the anatomical changes the face undergoes in order to offer the treatments that will provide the most optimal and natural result. It is not possible to achieve an optimal result with a single product, no matter how good it may be. Optimal treatment requires an understanding of the processes of facial change over the years and the ability to offer the widest range of treatments – both non-invasive aesthetic treatments and surgical procedures.

The face is composed of several layers, and the changes that occur during aging affect each of them. Generally, these changes can be divided into laxity and atrophy. Laxity mainly affects the skin layer, while atrophy affects the deeper layers – fat, muscles, and bones. Atrophy is often more difficult to identify and usually appears at earlier ages.

According to the changes that occur in the face, the correct combination of treatments can be offered. Today, the range of treatments is very wide and includes skin treatments such as chemical peels, mechanical peels, or laser resurfacing, improvement of skin texture using medical creams, substances that stimulate collagen and elastin production, threads, and more.

Dynamic wrinkles that result from facial muscle activity can be treated using botulinum toxin. Other wrinkles can be softened using fillers such as hyaluronic acid, calcium-based fillers, or autologous fat. Fillers, and particularly fat grafting, are excellent treatments for atrophy. It is important to understand the fat compartments in the face, their location, and their boundaries in order to perform filling in the correct anatomical layer and achieve optimal results.

In more advanced stages of facial laxity, or in specific areas such as the eyelids, eyebrows, jawline, or neck, tailored surgical procedures may be performed to improve these areas. The surgery may be performed under local anesthesia, sedation, or general anesthesia, depending on the extent of the surgery, the patient’s preference, and the medical condition.

In many patients, the first signs of facial aging appear in the eyebrow and eyelid area. Therefore, it is necessary to examine the position of the eyebrows and the condition of the eyelids before making any treatment decision. Understanding the changes in these areas allows the appropriate treatment to be chosen and prevents treatments that may not be correct (for example botulinum toxin in certain cases).

There are cases in which eyebrow lifting alone may provide the solution without the need for eyelid surgery, while in other cases the combination of treatments will provide the best result. There is also a difference between men and women in the natural position of the eyebrows, and this must be considered when planning treatment.

Eyelid correction and brow lifting procedures can be performed under local anesthesia and may be combined with other facial treatments such as filler injections, including fat grafting, or thread treatments.

The decision regarding the correct combination of treatments is made during a comprehensive consultation, in which all areas of the face are examined, the quality of the tissues is evaluated, the medical condition is assessed, and the patient’s wishes are considered.
